In Home Dementia Rehabilitation
Encara is proud to introduce the first in home Dementia Rehabilitation Program to residents in the Mornington Peninsula and City of Casey regions. The model is a comprehensive, evidence-based therapy and educational program to support and enhance consumers’ well-being and quality of life whilst living with Dementia. Encara’s team of Allied Health Professionals deliver a 12-week Dementia Rehabilitation program supported by non-clinical professionals, including music therapists and art therapists whom Encara engages for any adjunct treatment components. The program includes support and education for the consumer, care partners and family, acknowledging the different roles and challenges that each person may experience.
The Model of Care is derived from Encara’s existing Cognitive Support and Wellness Programs and includes a weekly Occupational Therapy home visit with an assessment conducted over the first two weeks and re-assessment on completion of the program. Weekly physiotherapy sessions deliver an individualised balance and strength training program. Encara’s approach to rehabilitation is complemented by holistic therapies such as massage, music and art therapy. The program includes referrals to other Allied Health professionals, which will be offered on an individual needs basis (e.g., Dietetics, Speech Therapy, Podiatry).

Encara accepts all referrals including self referrals, family referrals or health professional referrals including GPs, specialists, care partners and other Allied Health professionals; based on the following eligibility criteria:
Over the aged of 65.
Experiencing cognitive decline or have a dementia diagnosis.
Residents living in the communities of the Mornington Peninsula or City of Casey regions.
The program’s commencement is on the 17th of July 2023, and spaces are filling up fast!
